- Up-to-date Resume
- 2 legal IDs: Driver License, Passport, Permanent Resident Card or work eligibility IDs
- Social Security Card
- NY State License and Registration Certificates
- Two reference letters
- Physical Health exam results taken within the last 6 months with 1-step PPD
- Measles, Mumps, Rubella and Varicella (MMRV) lab works in the last 6 months
- Hep-B or option to sign our declination form
- Flu shot or option to sign our declination form
- Drug Screen in the last 12 months
- Course Completion Certificate/Diploma
- BLS/CPR Card
- Voided Check/ Direct deposit form for payroll
