MRT PERMANENT SUPPORT HOUSINGPOSITION DESCRIPTIONJob Description: Supporting Housing Case Manager/CounselorPosition Summary: The Permanent Supportive Housing Case Manager is primarily responsible for assisting individuals obtain and remain in permanent housing and mover towards self-sufficiency. Case Managers will network, collaborate and coordinate housing, behavioral health, substance and mental health services, community services and educational/employment services for participants in the program. This Case Manager also conducts individual and group sessions, documents individual participation, and submits required reporting data.Education Requirements: CASAC, Bachelor s or Master s degree in health and human services. Case Management and familiarity with working with individuals with co-occurring disorders is preferred.Experience Requirements: Experience working with individuals with substance abuse and/or mental illness, homelessness, familiarity with community agencies and entitlement programs, flexible scheduling during the week.Principal Duties and Responsibilities:Perform housing intake assessments to determine the needs of the participantAssist individuals in securing safe, affordable housing, develop relations with landlords and community relationshipsAssist with locating and securing safe, affordable housingPerform housing inspections, establish rental agreements and visits to ensure apartments are properly maintainedDevelop relationships with landlordsAssist participant in development of recovery plansProvide linkage to entitlement programs, medical, mental health and substance abuse treatmentProvide case coordination and collaboration with treatment programs and other community resourcesAssist and support participant s progress working with the employment specialist, training programs and employers to achieve education/vocational goalsMonitor participant s progress through regular home and office visitsUtilize harm reduction interventions and motivational strategies to provide client centered services that keep them in housingProvide direct care services including individual and group meetings, community meetings, and assistance with entitlement programsMaintain individual case records, collect data and reports as requiredMust have a valid driver licenseMust be available to work flexible hours, including on-call availabilityCome visit us at the Times Union Job Fair, May 19th from 10am to 2pm, 645 Albany Shaker Road, Albany NY 12205 candidates that can forward their cover letter, resume and references to Samantha ...@newchoicesrecovery.orgOur salaries are competitive and we offer a generous benefit package.
