Job Description:
- The position is held at Five Points Residential Mental Health Unit, which is part of the Office of Mental Health(OMH).
- OMH provides mental health services to forensic patients. Candidate will be required to submit of TB testing and recent flu shot.
- May have contact with incarcerated individuals. Candidate will be also subject to CNYPC orientation.
- Contract staff would mainly be working weekdays and assisting in providing therapeutic services to patients in a primary therapist role to include individual therapy and the completion of mental health assessments and documentation (Core History, Suicide Assessments/risk assessments, Weekly/Monthly Progress Notes, Treatment Plans, Discharge Plans), according to policy and procedure.
- Group facilitation will also be part of the expectations.
License Required:
- Possession of a license and current registration as a Licensed Master Social Worker(LMSW) in New York is required.
