Job Description:
Social Worker (Licensed) position on Staten Island!Full-time position, 35 hours/week!POSITION OVERVIEW:The CCBHC is comprised of several community based programs providing direct care to over 2000 clients annually, both on site and in the community. The Bridges to Care Social Worker reengages clients who have stopped attending treatment and are deemed a risk to harm self or others or an elevated risk for hospitalization. Working primarily in the field, the social worker provides in-home crisis stabilization/treatment services/supports and 24/7 emergency crisis response.KEY ESSENTIAL FUNCTIONS: Both in the CCBHC setting and in the community, the Bridges to Care Crisis Social Worker:
  • Conducts crisis evaluations to ascertain level of risk and uses evidence based interventions to manage risk
  • Uses motivational enhancement approaches to increase client (re)engagement in care and services
  • Assists clients with scheduling appointments for behavioral health, and physical health services and address barriers that may impact clients' ability to keep appointments, advocating for clients who need support
  • Participates in hospital discharge planning; communicates with service providers and family members to increase likelihood the client will successfully transition back to the community and engage in services
  • Maintain accurate and up-to-date case records e.g. progress notes, assessments, statistics, and other documentation as required by company and other governmental licensing and funding entities.
  • Attend provider meetings to promote collaboration and coordination of services
  • Participate in supervision and training activities
EDUCATIONAL/TRAINING REQUIRED:
  • Master's level degree in human services field with appropriate NYS licensure (e.g. LMSW, LMHC, LCSW)
  • Experience working with clients with severe mental illness and/or substance use;
  • Experience working as part of an interdisciplinary team;
  • Experience providing care management or care coordination in a medical or behavioral health environment;
  • Prior experience with electronic health records and Microsoft Office
WORK ENVIRONMENT/ PHYSICAL EFFORT:
  • The position entails providing services either in the office, in the community, as indicated (i.e., clients' homes hospital), or via telehealth
  • While the offices of the company are accessible in accordance with the ADA, the sites to which staff may need to travel may or may not be
  • To perform the essential functions of this job the candidate must be able to travel within New York City carrying equipment (notebook, forms, laptop, mobile hotspot, cell phone) weighing up to approximately 10 pounds
  • To perform the essential functions of this job, the candidate is routinely required to sit (60% of time) and stand (20% of time), and travel to and from appointments using public and private transportation {20% of time)
