Job Description:

Parker Jewish Institute is seeking a **Full Time Social Worker-Family Caregiver Coordinator** to join a five year grant-funded project under the Alzheimer's disease Caregiver Support Initiative.

**The Family Caregiver Coordinator is primarily responsible for:**

* Monitoring, tracking, follow-up and analysis of family and community services

* Monitoring and analysis of attendance and outcomes

* Providing referrals and conducting intake assessments in relation to the program

* Implementing innovative caregiver support services, including support groups, education and training programs, caregiver wellness programs and join enrichment opportunities

* Marketing and outreach of the program

The ideal candidate will be a Licensed Master Social Worker or possess a Master's degree in Social Work, possess two years of appropriate coordination or outreach experience, as well as experience working with the geriatric population and advanced proficiency in the use of Microsoft Office software.
