Job Description

New York City Health + Hospitals is the largest safety net health care system in the country and serves 1.3 million New Yorkers, nearly 400,000 who are uninsured. It provides medical and mental health services through its 11 acute care hospitals, four skilled nursing facilities, six large diagnostic and treatment centers and more than 80 community based clinics.

The Special Populations Clinics sites include three complex care models:

  • The Primary Care Safety Net Clinics (PCSN), serving high-need individuals experiencing homelessness with medical, social, and behavioral health needs.
  • Point of Re-Entry and Transition Practices (PORT), serving high-need individuals with a history of criminal legal system involvement, who are transitioning back into their communities.
  • Street Health Outreach and Wellness (SHOW), a street outreach program providing clinical support, behavioral health support, and basic needs to people experiencing street homelessness. SHOW teams are tied to brick-and-mortar PCSN clinics.

Special Populations Community Health Workers (CHW) are integral members of the care teams across these settings. The Roving Program Manager will supervise the facility-based Special Populations CHW Supervisors at select sites.

Summary Of Duties And Responsibilities

The Roving CHW Program Manager, Special Populations will provide critical oversight for CHW Supervisors and CHWs at their designated Special Populations Clinic sites, including the Primary Care Safety Net Clinics and Point of Re-Entry and Transition Clinics. Within each site, one or more CHWs will provide care to patients to improve their health, strengthen their connection to primary care, and achieve patient-driven goals. The Roving Program Manager will meet regularly with each CHW team at their assigned sites to review program progress, provide coaching and mentoring, and identify issues for escalation to the respective facility and Central Office program leadership. The Roving Supervisor will be responsible for regular supervision of CHW Supervisors at Special Populations Clinics, including rotations between six clinical settings regularly at multiple acute care facilities and associated community settings. The Roving Program Manager will report directly to the Central Office Program Team responsible for Program Oversight. The Roving Program Manager is expected to provide case management expertise, conduct regular case reviews, address personnel issues, and participate in clinic team operational meetings. They will work in collaboration with a facility-identied individual responsible for their team s timesheets and daily task management/work assignments.

General Tasks And Responsibilities Will Include

  • Provide guidance to teams of CHWs and CHW Supervisors at multiple Special Populations clinic sites in adult primary care settings serving patients experiencing homelessness and/or patients re-entering the community from carceral settings.
  • Meet regularly with assigned site-based Special Populations CHWs, CHW Supervisors, and clinical leads to review key program metrics including patient enrollment targets and achievement of patient goals, training needs, and best practices.
  • Meet regularly with the Central Office Care Models team to share program progress, discuss training and coaching needs, determine opportunities for improvements to the program model, and share challenges and best practices for Special Populations CHWs.
  • Establish strong relationships with the clinical, patient support, and administrative teams at the assigned sites to ensure all Special Populations CHWs and Special Populations CHW
  • Supervisors are able to access needed patient supports.
  • Present Special Populations program model, goals, design, and metrics to leadership and staff to promote program buy-in; field requests/questions from staff and respond or delegate to
  • Special Populations CHWs and CHW Supervisors as appropriate.
  • Escalate clinical and administrative challenges identified by the Special Populations CHWs and CHW Supervisors to collaboratively identify and implement solutions.
  • Liaise with key community partners to ensure available community resources and processes to access are clearly defined and disseminated to Special Populations CHW Supervisors and CHWs.
  • Identify, initiate, manage, and foster trusted relationships with community organizations and local stakeholders in coordination with PORT, PCSN, & SHOW teams to integrate Special
  • Populations CHW activities with the local community.
  • Create focused community partnerships in line with Special Populations CHW program goals and the social, cultural, and linguistic needs of the patient population.
  • Continuously expand knowledge and understanding of community resources, services, and programs by attending community meetings, health fairs, and community advisory committee meetings to understand community issues or build relationships with community members.
  • Provide technical assistance to community partners to implement referral workflows and technology (e.g. social service referral platform use).
  • Collaborate with the Data Analyst and other members of the Central Office Care Models team to develop reports and presentations with the goal of communicating program progress to internal/external stakeholders.
  • Collaborate with Special Populations CHW Project Manager, Special Populations CHW Program Director, and other members of the Central Office Care Models team to identify and respond to training needs; track and report on training participation; conduct performance improvement activities.
  • Represent the Program at meetings, events, and functions as needed.

Minimum Qualifications

  • A Baccalaureate degree from an accredited college or university with a major in Social Science, Social Work, Public Health or related discipline and three (3) years of full-time, progressively responsible experience in the field of community relations, community organization, public health and/or social services in a public or private social or health agency; or
  • A satisfactory equivalent combination of training, education and/or experience. However, all candidates must have minimum of a four-year high school diploma or its educational equivalent approved by a State s Department of Education or a recognized accrediting agency.

  • Experience in a clinical setting
  • Strong computer skills (MS Office Suite)
  • Comfort with and ability to analyze and interpret data
  • Excellent communications skills, including written and oral presentation skills, with the ability to engage, build credibility and engender trust across all levels of collaboration
  • Ability to communicate and collaborate with team members from multiple disciplines and at multiple levels
  • Detail-oriented, flexible and professional
  • Demonstrates initiative, sound judgment, flexibility and creative problem-solving skills
  • Excellent time management skills, ability to work longitudinally in multiple settings with different needs and to manage distinct projects with overlapping timelines

Preferred Skills

  • Experience with or comfort in learning Electronic Health Records
  • Experience managing staff across multiple settings
  • Experience with New York City's healthcare industries, including major hospital systems, other significant providers, city and state agencies, and/or relevant community-based organizations and agencies
