- New York License and registration (RN, LPN, CNA)
- Two reference letters
- Physical exam with PPD, MMRV and Hep B lab works in the last 10 months
- Drug Screen (10 Panel - within the last 10 months)
- Flu Shot
- School diploma or completion (RN, LPN, CNA)
- CPR (RN, LPN & optional for CNA)
- Malpractice Insurance (RN, LPN & optional for CNA)
