Primary Duties & Essential Functions:Care of Individuals:
- Manage and implement a program, which provides an environment to carry out individual's role in the overall treatment program
- Coordinates and provides a therapeutic interactionthrough experimental methods of learning.
- Safely transports individuals as assigned per level of supervision required.
- Assist in personal care of individuals.
- Assures appropriate boundaries.
Policies and Procedures:
- Learns and practices agency policies and procedures.
- Learns and implements each individual treatment plan including: physical interventions, documents and makes proper notifications according to procedures.
- Provides coverage as assigned.
- Remains on-duty until properly briefed and appropriate parameters are met.
Supervision:
- Supervises and coordinates all documentation relating to Recreation activities including Assessments, Progress Notes, Attendance Sheets, Program Activity Sheets, and Comprehensive Treatment Plan Narratives.
- Participates in agency and other staff development opportunities as assigned.
- Attains and actively participates in all assigned trainings.
Agency Responsibilities:
- Engages in all living unit, service, and/or department planning and goal attainment.
- Participates in activities and committees.
- Changes work location to meet agency needs.
- Maintains appropriate attendance and punctuality.
Communication:
- Communicates regularly with other personnel through the facilitation of individuals staff supervision
- Completes all necessary communications, documentation's, and loggings.
- Properly notifies and/or consults with the Activities Coordinator
- Proactively and effectively liaisons with other disciplines, internal and external Service Providers and families as appropriate.
- Appropriately utilizes chain of command
- As assigned and will complete section of monthly report.
Environment of Care:
- Completes assigned cleanup procedures, checklists and schedules.
- Proactively and effectively liaisons with Maintenance, House Manager, Coordinators, Direct Care Supervisors and Directors.
- Supports Quality Assurance Quality Improvement directives
Other Responsibilities:
- Completes other responsibilities as properly assigned.When required, assures and assists with supervision of all individuals served.
- Promotes and supports cultural diversity and competence and is strength based.

Required Education, Knowledge and Skills:
- Minimum High School Diploma/GED required, Associate's degree preferred.
- Valid driver's license and clean MVR to ensure ability to drive agency vehicles.
- Medication certified within six months of hire.
- Various hours/shifts. Full time.
- Must be available to work evenings, weekends and holidays as required.
- Ability to write legibly.
- Ability to communicate effectively with other disciplines and individuals served.
