Parker Jewish Institute Hospice and Home Health Care is seeking compassionate and motivated **Community Physical Therapists** to join Parker's Certified Home Health Agency (CHHA).
Parker Jewish Institute Hospice and Home Health Care offers the finest in home health and hospice services from an interdisciplinary team of health care professionals. Parker Jewish Institute for Health Care and Rehabilitation is a not-for-profit adult health care system comprised of multiple health services servicing the Metropolitan New York Area, including Long Island, Brooklyn, Queens, Bronx, Manhattan and Westchester.
The Physical Therapist at Parker Jewish Institute's Certified Home Health Agency is responsible for treatment and delivery of Physical Therapy services to home care patients. They are also responsible for the coordination of services through the Nurse Coordinator and for timely documentation.
This position requires the full understanding and the active participation in fulfilling the mission of Parker's Certified Home Health Agency to deliver exceptional care and to enhance the quality of life of our patients.
**QUALIFICATIONS:**
* Graduate from Physical Therapy curriculum approved by the American Physical Therapy Association of the Council on Medical Education of the American Medical Association
* Minimum of 1 year experience as a staff Physical Therapist
* Prior experience in a community setting strongly preferred
* Tech savvy, with proficiency in AllScripts or another electronic medical record (EMR)
* Access to reliable transportation a must
* Fully Vaccinated against COVID-19
