  • Engage patients through one on one relationship building, treatment education, advocacy and support.
  • Provide mentoring, emotional support and a buddy system to patients improving the continuum of care.
  • Empower patients with PrEP education, keeping PrEP follow-up appointments, rescheduling missed appointments, communicating with provider and antiretroviral adherence.
  • Act as a liaison between the patient and the PrEP Specialist and the Provider.
  • Assist in running group sessions, as directed by the PrEP Specialist.
  • Submit timely AIRS data documenting patient activities.
  • Participate in street outreach activities, under the supervision of the PrEP Specialist.
  • Participate and assist in organizing community events, i.e. World AIDS Day, PrEP Awareness Week, Pride Month, National HIV Testing Day, etc.
  • Attend monthly educational forums.
  • Attend weekly supervision.
  • Attend and participate in monthly team meetings, department meetings, and other required meetings.
  • Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Family Healthcare Policy and Procedure Manual and Employee Handbook.
  • Maintain confidentiality of all aspects of Cornerstone Family Healthcare including, but not limited to, patient confidentiality, financials, and employee relations.
  • Perform other duties as assigned.
Requirements
  • Knowledge of HIV/AIDS and PrEP.
  • Must have a cross-cultural sensitivity and ability to communicate with people of diverse backgrounds.
  • Must be able to maintain a non-judgmental attitude.
  • Must be well organized and attentive to detail.
  • Must be able to maintain strict confidentiality when working with populations at risk for HIV and people living with HIV.
  • Must have direct lived experience of being on PrEP Consumer.
  • Travel/Time Requirement: Must have a valid, unrestricted driver's license and independent means of transportation for frequent travel to locations throughout the assigned county as well as to the offices of Cornerstone.
PLEASE NOTE THAT THIS POSITION IS ONLY 1 DAY FOR 7 HOURS A WEEKSalary Description $15.00 per hour
