**Position:** Nutritionist **Location:** New York, NY **Job Id:** 152 **# of Openings:** 1 **JOB SUMMARY** Under the direction of the Chief Executive Officer, the incumbent in this position is responsible for assessing patients' nutritional needs, developing andimplementing care plans in coordination with the patients provider and other members of the health care team, oversight over the agencys nutrition and wellness programs. Incumbents in this job may have access to proprietary consumer or company information and must maintain the privacy and confidentiality of this information. All employees must perform their jobs with a focus on risk management, occupational safety and health, and providing internal and external customer service. Employees working in a supervisory capacity must practice effective human resources practices to ensure a healthy working environment. **KEY JOB DUTIES AND RESPONSIBILITIES** Incumbents in the job must be able to perform the key duties and responsibilities of the job and all other duties as assigned with or without reasonable accommodations.

* Provides individual and groups counseling on nutrition to prevent/manage disease and promote health.

* Evaluates individual patient nutritional needs, develops nutrition care plans and provides medical nutrition therapy utilizing evidence-based practices for a range of health conditions including diabetes, cardiovascular disease, obesity, and gastrointestinal diagnoses.

* Ensures appropriate documentation of nutrition assessments, care plans and recommended interventions in the patients chart; reviews the documentation of others regarding nutritional concerns and responds appropriately.

* Assesses patients nutrition status including lab value interpretation, patient eating patterns, weight history, food/drug interactions, and substance use.

* Order and create patient education materials on healthy eating and active living.

* Provide group and individual nutrition counseling to students and parents at the School Based Health Center.

* Act as a liaison/advisor to the medical staff and the school based health center staff as requested.

* Coordinate, plan and conduct education programs on nutrition for agency staff (including Employee Wellness Program) and the community at large.

* Coordinate, plan and implement National Diabetes Prevention Program (NDPP) for prediabetic and overweight patients.

* Develops strategies to expand nutrition programming and create new initiatives for patients.

**SUPERVISORY RESPONSIBILITIES** This position does not have supervisory responsibilities over subordinate staff. **REQUIRED KNOWLEDGE, SKILLS, AND COMPETENCIES**

* Two to three years of supervised practical experience working in ambulatory care settings in local community health centers;

* Demonstrable experience as a Registered Dietitian

* Bi-lingual Spanish preferred;

* Knowledge of current procedural terminology coding (ICD-10 codes) as required by third party payers;

* Provides direction to medical students, interns, or residents assigned for training;

* Experience with electronic health record systems;

* Excellent written and verbal communication skills are required.

**SPECIAL REQUIREMENTS**

* Must be a graduate of an accredited dietetics bachelors or masters degree program

* Must be a Registered Dietitian (RD)

* Must be a Certified Diabetes Educator (CDE) in New York State

* Certified Dietitian Nutrition (CDN) in New York State

* National Provider Identification Number (NPI)

**WORK ENVIRONMENT** The Nutritionist is assigned to work at the health center.
