A major hospital in Brooklyn is looking for a social worker for a full time temporary position for discharge planning and other services as needed.

Job Description :

Ideal candidate should be outcome driven with outstanding interpersonal skills and ability to cultivate effective relationships in an interdisciplinary, fast-paced environment.

Ability to excel in fast-paced and outcome driven environment preferred.

The ability to communicate and work collaboratively in a multidisciplinary setting required.

Flexibility, compassion and ability to work independently are also desired.

Key responsibilities includes Discharge Planning, Biopsychosocial Assessment, use of Community Resources and Care Coordination.

Requirements/Qualifications :

Must possess LMSW Licensed by New York State.

Experience in medical acute care setting is preferred including inpatient and outpatient social work.

A solid knowledge of community based referral sources.

Excellent organizational, critical and independent thinking skills required.

Bilingual (English/Spanish) highly preferred.
