A health services network in New York City is currently seeking a licensed Registered Nurse (RN) to join their staff in Manhattan as a Hospice Referral Coordinator.Responsibilities: The Hospice Referral Coordinator (RN) will be responsible for:
- Evaluating and establishing a plan of care for patients referred for home health services
- Facilitating admission of referrals upon discharge from the hospital to a home care program that meets the needs of the patient and family members
- Developing and maintaining positive relationships with Physicians, Social Workers, Case Managers and Discharge Planners
- Performing other duties, as needed
Qualifications:
- 2+ years of experience in Home Care Nursing
- Graduate of an accredited School of Nursing
- NYS Registered Nurse (RN) license
- Proficiency in Physical Assessment and Interviewing skills
- Solid assessment, clinical, and documentation
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
