Home Health Aide Home Care Division **Description**
The Home Health Aide provides personal care and assistance in activities of daily living to Home Care patients according to the Plan of Care developed by the Community Health Nurse and/or Rehabilitation Therapist. The Home Health Aide is an integral member of the Home Care team.
**Qualifications**
High School Diploma or GED required. Home Health Aide Certificate or successful completion of the NYS approved . Home Health Aide Training Program. CHRC- Eligibility for employment validated by Criminal History Record Check . A valid NYS Drivers License Must have own car, valid NYS Auto Insurance, and telephone
