Habilitation Specialist - ILEARN ProgramRate of Pay : $16.32 - $17.95 (depending on experience)PART TIME Position- Variable hours, 4-5 hour shifts, evenings and weekends. Driver's License and personal vehicle required.QUALIFICATIONS: HS diploma/GED, valid NYS Driver's License with a clean driving record.BASIC FUNCTIONS:Model and provide instruction, services and supports to program participants during evening and weekend I-LEARN recreational program. This could include, but is not limited to:
  • community integration
  • community travel and transportation
  • money management and budgeting
  • appropriate socialization/interactions
  • healthy lifestyle
  • health and medical issues,
  • and any skills needed to enable the individual to access the community as independently as possible.
  • Other instruction, services and supports as indicated by your supervisor.
Modeling, instruction, services and supports are to be based on the participant's valued outcomes, which are uniquely tailored for and by each individual participant at their bi-yearly Life Plan Meeting. These services and supports are designed to enhance the quality of life for all Beyond Support Network participants.RESPONSIBILITIES:
  • Facilitate outings in the community and at the Main Office in order to promote new experiences and meeting new people.
  • Ensure that each participant's programmatic Staff Action Plan is implemented as written.
  • Provide input for participants as requested by supervisor for review at bi-yearly Life Plan Meeting.
  • Follow agency and OPWDD regulations, policies and procedures related to participant rights, grievance and emergency contacts.
  • Participate in required agency trainings, as directed by your supervisor.
  • Attend your departments' and agency staff meetings as directed by your supervisor.
  • Complete necessary paperwork and reporting documents in a timely manner; as directed by your supervisor and the Beyond Support Network employee handbook, (Paperwork and reporting documents include but are not limited to: time sheets, mileage, schedules, I-LEARN notes, billing forms, incident reports, other forms or paperwork as directed by supervisor).
  • Perform all duties as assigned by your supervisor according to the mission statement of Beyond Support Network.
