Perform within the guidelines of The NYC Health + Hospitals/ Queens, and The New York State Department of Health WIC Program standards and implements The NYSWIC System (NYWIC).2. Determine eligibility of WIC applicants using the NYWIC System and the nutritional risk criteria set up by the NYS Department of Health WIC Program.3. Certify and enrolls eligible applicants into Queens Hospital WIC Program:a. Record and interprets laboratory, physical and social data in NYWIC.b. Conduct onsite anthropometry measurements.c. Perform participant-centered nutrition assessment and education through interviews and explains WIC benefits to clients or caretakersd. Screen all pregnant women for cigarette smoking, alcohol, drugs and other substance abuse; counsel and monitor clients for nutritional needs and evaluate nutritional care and care planse. Screen participants at certification for high risk status and other nutritional risk and implement care plan using the New York State Department of Health WIC Program's Nutritional Risk Criteria and High-Risk Criteria Lists.f. Refer participants to other health professionals/services or social services as neededg. Issues, void and re-issues WIC food packages in NYWIC to active participantsh. Consult physician about appropriate Special Supplemental Foods for participants with special needsi. Track participant's progress, and document outcomes.j. Schedule return appointments.4. Provide group or individual-centered nutrition education to participants at initial, secondary, and high risk contact. Document intervention in NYWIC and/or manual certification forms5. Perform (VOC) Transfer In or Transfer Out at participants' or other Local Agency's request.6. Implement Farmer's Market Nutrition Program.7. Promote a positive attitude conductive to the workplace, which enhances and further the Mission, Vision and Value statement.8. Meet all compliance standards/ required such as annual physical evaluation/ assessment, Annual In-Service training; Fire Drill participation, etc.9. Participate in performance improvement Activities as needed.10. Be responsible for enhancing your personal professional development through attendance at outside and NYC Health + Hospitals/Queens.11. Be responsible to represent the WIC program professionally, at WIC program meetings, workshops, health fairs and community events.12. Exhibit flexibility within the NYC Health + Hospitals/Queens, as it pertains to work assignment, work place; site designation and tour of duty.13. Performs alternate hours' rotations as assigned.14. Adhere to the Vision/Mission and Value Statement of the Organization in all endeavors.15. Perform other duties as assigned by the WIC Assistant Director.Minimum Qualifications1. Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or,2.
