Job Description:
Position Summary:The clinician is responsible for providing substance abuse treatment interventions and services to patients throughout the treatment process. This includes; communicating with referents, facilitating groups, family and individual sessions, case management supervision, data collection and documentation, referrals to other agencies, conducting assessments and monitoring progress on treatment plans, and working collaboratively with the treatment team and community providers.Education Requirements:Minimum of a Master s degree, having additionally secured licensure and clinical status (LMSW/LCSW) from the New York State, Division of Professional Licensing Services or equivalent credential (LMHC), in a related field or CASAC preferred.Experience & Related Requirements:Clinician will poses a minimum of three years employment experience providing substance abuse treatment and interventions. Knowledge and ability to provide evidence based interventions for individuals, families and group practice including; Holistic Health Recovery Program, Motivational Interviewing, Community Reinforcement and Family Therapy and Seeking Safety. Strong writing and communication skills required. Knowledge of local community resources, related service agencies and managed care representatives is a plus. Experience working with individuals with a co-occurring substance abuse and mental health diagnosis is a plus. Ability to work with a diverse population along with a working knowledge of cultural competencies is preferred. Candidates must have computer skills, reliable access to transportation and be willing to travel, available to work full-time (37.5 hours/week), flexible schedule given that evening or weekend encounters are likely.Principal Responsibilities:Conduct assessments, diagnosis and determine appropriate patient level of care in accordance with LOCADTR criteria.Provide individual, family and group therapy. Work with client and treatment team to assess and prioritize patient needs. Provide crisis management interventions as warranted. Maintain patient care documentation within the approved program format and document incidents and patient care issues in accordance with OASAS standards and agency procedures.Develop treatment plans in concert with the client based on identified needs and treatment goals. Include when appropriate the family, other health care professionals and/or other community agencies in treatment plan development. Regularly update treatment plans and revise as needed in conjunction with the client and treatment team. Facilitate the discharge planning process, making appropriate referrals and follow up at the conclusion of treatment.Monitor the client for the appropriate level of care based on established criteria and progress in treatment. Ensure that all necessary documentation accompanies changes in treatment and referrals to outside agencies. Ensure the client understands and is in agreement with changes in the level of treatment.Comply with agency policies and procedures. Attends trainings, seminars, and in-service trainings to increase skill level and maintain credentials.Contribute to the achievement of organizational goals.
