**A Brief Job Description:**

Catholic Charities Community Services has a Childrens Health Home Care Manager position open in Rochester, NY. This Childrens Health Home Care Manager position will provide intake and ongoing care management to individuals within the Childrens Health Home program living in Monroe, Finger Lakes or Southern Tier Regional Counties, supporting and coaching youth and families with navigating multiple complex systems.

Past Childrens Health Home Care (CHHC) experience is preferred and at least 2-3 years of Human Services paid experience and due to funding a candidate MUST have a Bachelors Degree or higher at this time.

SPECIFIC DUTIES:

1. When needed, serve as contact for referrals and conduct necessary screening and/or follow up for client to proceed with intake in applicable program(s).

2. Provide youth-guided and family-driven care management services to caseload assigned, maintaining accurate and verifiable documentation of all services rendered.

3. Using a person-centered approach, complete comprehensive and detailed assessments, reviews, crisis plans, and care plans as required by standards set by DOH and lead Health Home contractors.

4. Complete CANS-NY assessment for clients assigned and ensure a comprehensive view of the client/family strengths, challenges, skills, supports, etc.

5. Demonstrate, promote and practice cultural competency towards clients through respect and understanding.

6. Recognize and embrace diversity through supporting and encouraging the strengths of each individual youth and family served.

7. Provide a meaningful core service for each client assigned, which includes face to face visits per month based on need and acuity level, to ensure all care plan goals are addressed to the satisfaction of the clients served.

8. Utilizing a team approach, maintain accessibility for clients during the workday.

9. Assist youth and families in the acquisition and maintenance of public benefits e.g., Medicaid, SNAP, Social Security, etc.

10. Provide linkage/referrals and coordination of resources/support for medical care, mental health, social, recreational, and housing/basic needs, etc.

11. Remain current in the field, support and implement best practice service models within the program.

12. Make independent decisions and recommendations regarding immediate risk factors presented and provide crisis intervention and follow-up as needed.

13. Provide education to youth and family concerning the nature and desired effect of services.

14. Assist youth and families in managing and planning for safety needs and creating individualized safety/support plans.

15. Demonstrate flexibility to be available to clients and families in their homes/communities including working outside of normal business hours as needed.

16. Provide on-call availability via cell phone to the Health Home program as needed.

17. Serve as client advocate.

18. Facilitate the development of a Care Team to carry out individualized and strength-based Plan of Care through regular case conferences and case reviews.

19. Participate in staff meetings and other Care Coordination department activities.

20. Ensure compliance with all applicable local, State, Federal and Health Home Network regulations and agency policies.

21. Participate in mandatory ongoing training and professional development offered.

22. Represent Catholic Charities Community Services on appropriate boards and committees as assigned by CC Director.

23. Other duties as assigned.
