Job Type Part-timeDescriptionCase Managers provide community-based support to individuals and/or families who have experienced homelessness. Case Managers work with people in their homes, utilizing a housing first approach, understanding people who have experienced chronic homelessness require additional support to achieve positive tenancies. This position provides on-going case management support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of participant needs.ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Provide case management services and resources to individuals who are experiencing homelessness creating a pathway to employment, vocational training, and sustainability.
  • Evaluation and assessment of referrals, utilizing a housing first framework and understanding the barriers to house people experiencing homelessness.
  • Work with community providers to ensure adequate and stable housing for program participants.
  • Create and implement participant-centered individual action plans (IAP) within the required time frame with an emphasis on participant goals.
  • Collect and maintain data-based statistical information, maintain accurate case records compliant with federal, state, local and agency policies, procedures and regulation.
  • Monitor participant progress and complete assessments in order to identify participant's strengths, needs, barriers and goals in order to sustain housing and increase self-sufficiency.
  • Facilitate resident trainings including employability workshops, budgeting, communication skills, job search training and assistance and etc.
  • Reports to the supervisor all pertinent information concerning illnesses, accidents, untoward events, staff difficulties, repair needs, etc.
  • Network and maintain positive working relationships with local landlords, property managers, housing authorities, social service organizations and state agencies.
  • Effectively performs problem solving functions, document and report crisis situations, staff difficulties, repair needs, etc.
  • Provides support to SRO and Gaslight Village programs as needed.
  • Maintains client confidentiality at all times.

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.RequirementsEDUCATION and/or EXPERIENCE Required: Associate degree in the Human Services field and two years Human Services experience.Preferred: Bachelor's degree in human services field and 1-year Human Services experience, particularly with developmentally delayed, geriatric and co-occurring disorder clients.Salary Description $18.25 per hour
