Case Manager for Minor Victims of Trafficking

Program Description: Our intervention program is dedicated to equipping juveniles navigating the intersection of human trafficking and street life with the education, skills, and support necessary for sustainable life transformation. Our comprehensive program encompasses mental health services for both youth and their families, personalized case management, mentorship opportunities, hands-on experiential learning, and peer support and advocacy initiatives.

Position Description: We are seeking a community-oriented case manager dedicated to enriching the lives of young individuals and empowering them to embrace positive life trajectories. The ideal candidate will demonstrate a fervent commitment to serving a demographic predominantly comprised of at-risk juveniles, youth affiliated with gangs, and victims of human trafficking aged 12 to 21, along with their families. As a case manager, the primary responsibilities will involve offering assistance and connecting these youth and families with essential support services and resources.

Responsibilities:

Provide victim-centered, trauma-informed intensive case management and advocacy services to victims of human trafficking both on-site and in the field. This includes conducting client assessments, developing comprehensive service plans, regularly reviewing progress throughout treatment, and advocating for clients within various systems such as courts, schools, and human service agencies. Additionally, determine eligibility for other public or community-based programs.

Assist survivors in navigating their rights as crime victims across different systems, advocating on their behalf when necessary.

Lead life skills training group sessions for both youth and parents/caregivers.

Conduct one-on-one counseling and crisis intervention sessions with youth and their family members.

Organize and civic engagement opportunities and experiential mentoring trips, activities, and events.

Gather collateral information from participants, family members, and relevant external sources to assess and monitor behavioral health needs.

Make effective referrals to additional resources such as integrated mental health and substance abuse services, psychiatric care, and medical services.

Cultivate and sustain consultative relationships with schools and other agencies involved with the youths and families.

Participate in relevant trainings and weekly team meetings to contribute to coordinated services for victims of human trafficking.

Represent the organization at pertinent task forces and collaborative efforts, enhancing the region's capacity to deliver trauma-informed, survivor-centered, and culturally rooted services.

Collaborate with STRONG intervention teams and partnering agencies to coordinate parent and community events focused on trauma and treatment modalities for victims of human trafficking.

Maintain thorough and timely documentation of all interactions with and information about the youth.

Perform any other duties as assigned.

Qualifications:

The Specialist should possess a strong passion and expertise in trauma-informed approaches and considerations, including chronic trauma theory and advocacy for youth empowerment.

BILINGUAL IN SPANISH HIGHLY PREFERRED

Bachelor's degree in a health and human services field (e.g., social work, psychology) is required. Graduate degree preferred.

LMSW or licensure in a related human service field preferred.

A minimum of four years of post-graduate experience as a community professional providing services to survivors of violence and facilitating groups with individuals and families.

Ability and willingness to deliver training and professional development to staff and interns as necessary.

Knowledge of relevant community resources and staying up-to-date on news affecting the youth and families we serve, particularly victims of human trafficking.

Exceptional interpersonal skills, with the ability to work autonomously and collaboratively within a team environment.

Capable of establishing rapport with youth while maintaining professional boundaries.

Comfort in engaging with families, community members, and the diverse systems our youth and families interact with.

Strong organizational skills, capable of prioritizing tasks, taking initiative, managing multiple assignments, and meeting deadlines with limited direct supervision.

Meticulous attention to detail and ability to perform effectively under pressure.

Dedication to excellence and a willingness to learn and grow professionally.

Commitment to maintaining the confidentiality of records and information.

Demonstrated proficiency in computer skills.

Possession of a valid NYS Driver's License and access to a motor vehicle is required.

The position is guaranteed through Dec. 31, 2024.

Salary: This is a full-time position. Commensurate with educational and professional experience. ($52k to $55k).

Days & Hours: Full-time. Must have a flexible schedule. Mainly late morning into evening hours. Weekends as needed.
