Job Description:

A New York City-based private organization is looking to fill an immediate need on their Queens-based team with the addition of a new Case Manager III. Under the direction of the Supervisor, the Case Manager III will be responsible for providing case management services, assessment and intake of participants to determine eligibility for services.

About the Opportunity:

  • Start Date: ASAP
  • Assignment Length: Ongoing (No Designated End Date)
  • Schedule: Monday to Friday
  • Hours: 9am to 5pm (1hr unpaid lunch)

Responsibilities:

The Case Manager III will:

  • Advocating, linking, and escorting services for participants to prevent homelessness.
  • Providing and/or assure that each case assigned receives regular visits and contact, as needed
  • Establishing and maintaining case records, including completing progress notes
  • Ensuring all relevant case related information is inputted into case files and corresponding databases
  • Assessing the needs of the whole consumer/family including their basic needs such as food, clothing and shelter
  • Following-up on referrals you make on behalf of a consumer/family and responding to all referrals made to the program
  • Performing other duties, as needed

Qualifications:

  • Master's Degree in Social Services or related field
  • Computer savvy
  • Solid assessment and documentation skills
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized

Desired Skills:

  • Bilingual (English and Spanish)
  • Experience providing direct services to at-risk populations or individuals/families previously in shelter
