A social services agency in New York City is looking to add a Spanish-speaking professional to join their growing Comprehensive Program as a Case Manager. Working under the Program Supervisor, the Case Manager will be responsible for both clerical and practical support.
Responsibilities:The Case Manager will:
- Enroll 50 participants into the program and lead and manage 50 cases independently
- Conduct timely intake, orientation, and assessment of all participants in order to develop service plans to help their families gain confidence, self-sufficiency, and greater connection to their community
- Provide advocacy, outreach, and referrals to entitlement agencies, social service providers, and other programs on behalf of program participants
- Perform related crisis intervention activities and escort participants to appointments as needed
- Develop ISP plan with short- and long-term goals for participants enrolled in the program
- Develop self-advocacy goals with participants enrolled in the program
- Perform follow-ups on bi-weekly basis.
Qualifications:
- Bachelor's Degree in Human Services or relevant field required or completion of the Family Development Credential (FDC) Program within the first 6 months of employment
- Bilingual (English and Spanish)
- Great interpersonal skills
- Excellent communication skills (written and verbal)
Desired Skills:
- 1+ year of Case Management experience in a Community-based setting
- Counseling credentials
