Job Description:

A social services agency in New York City is looking to add a Spanish-speaking professional to join their growing Comprehensive Program as a Case Manager. Working under the Program Supervisor, the Case Manager will be responsible for both clerical and practical support.

Responsibilities:The Case Manager will:

  • Enroll 50 participants into the program and lead and manage 50 cases independently
  • Conduct timely intake, orientation, and assessment of all participants in order to develop service plans to help their families gain confidence, self-sufficiency, and greater connection to their community
  • Provide advocacy, outreach, and referrals to entitlement agencies, social service providers, and other programs on behalf of program participants
  • Perform related crisis intervention activities and escort participants to appointments as needed
  • Develop ISP plan with short- and long-term goals for participants enrolled in the program
  • Develop self-advocacy goals with participants enrolled in the program
  • Perform follow-ups on bi-weekly basis.

Qualifications:

  • Bachelor's Degree in Human Services or relevant field required or completion of the Family Development Credential (FDC) Program within the first 6 months of employment
  • Bilingual (English and Spanish)
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)

Desired Skills:

  • 1+ year of Case Management experience in a Community-based setting
  • Counseling credentials
