**Department & Position Overview:**

The Assistant Manager of Forms in our Intake department will have the opportunity to build, streamline and manage new processes that create operational efficiencies to our patient and caregiver experience. The (role) will work with salesforce developers and business partners to take decisions of software solutions, organizational structure, and workforce management. This is an exciting opportunity to lead a newly centralized team.

**First 30 days:**

* You will gain an understanding of the patient experience and forms processing and procedures across the organization through an extensive 2 month training.

**First 60 days:**

* You will create a centralized Forms team and assess the staffing needs for this team. All team-specific forms teams will join the Central Forms Team to use the new system and provide feedback for areas of improvement.

* Train new and existing new employees on new and improved forms processes

* This role will work on maintaining the team and system to ensure quality and service standards are being met, through audits, continued education, and collaboration with internal and external stakeholders

**First 90 days:**

* Partner with salesforce developers to build a systematic process of how patients and caregivers are notified about the need for additional paperwork and how that paperwork will be processed and logged in our internal systems.

* Meet with health plans to understand open communication and understand optimal solutions upload and best response times.

**Ideal Candidate Will Possess:**

* Strong attention to detail; excellent time management and organization skills

* Experience in process improvement - experience streamlining processes to enhance efficiency

* Excellent technology skills in salesforce

* Excellent communication skills

* Passion for exceptional customer experience

* Ability to lead a team and create new processes for a newly formed department

This role is based out of one or both of our two main offices in New York, NY or Lake Success, NY. For now, we are entirely remote until it is completely safe for our employees to be back in the office.
