***Roles & Responsibilities:***

* Patient Access Services needs to maintain patient flow throughout the medical center. Under the supervision of an Admitting Manager/Supervisor, ASRs are responsible for all duties relative to the registration, admission, transfer and discharge of all patients at Mount Sinai Beth Israel. Updates and maintains census information on all patients. Collect Birth/Death/Spontaneous Termination of Pregnancy certificate information and submits to DOH within the specified time. Is cognizant of the philosophy, standards, objectives and policies of the department and the Hospital Center. Provide excellent customer service during all interactions with internal and external customers. Accurately enter patients information into the hospitals registration system.

Duties and Responsibilities:

1. 1. Demonstrates the ability to perform service/administrative tasks.

2. 2. Greets patients/family in a courteous and caring manner.

3. 3. Answers phones promptly and courteously by identifying self and the department, and responds to all emergency requests.

4. 4. Listens closely to all customers requests and concerns and follows-through or refers to a supervisor, if appropriate.

5. 5. Performs appropriate name search in ADT to eliminate duplication of Medical Record Numbers.

6. 6. Obtains/confirms complete and accurate demographics and insurance coverage by interviewing the patient and entering information in appropriate system. Secures additional information from physician and/or patient. Verifies that data is correctly entered in appropriate system and that addressograph plate/face sheets are made.

7. 7. Processes all support documentation and forms to compete admission. Explains accommodations and required forms to patient, assuring completeness. Obtain signatures as related to hospital policies and procedures.

8. 8. Reconciles Daily Census in ADT and TeleTracking. Inputs accurate data and distributes daily to Census Distribution List if directed.

9. 9. Timely and accurately assigns beds for all admissions/transfers and direct admissions based on patient diagnosis and bed availability utilizing TeleTracking (BedBoard electronic systems). Enters/updates appropriate accommodation code(s) and appropriate Attending.

10. 10. Monitors transfer appropriately and accurately and communicates with appropriate receiving team. Discharges/closes inpatient and ambulatory surgery cases using appropriate code(s) immediately upon request.

11. 11. Enters expiration/TOP information in log book and appropriate system; contacts Organ Donor Network and assures Electronic Death Certificate is complete, signed and printed.

12. 12. Notifies appropriate personnel of a patient/newborns admission/transfer and may escort to appropriate unit.

13. 13. Monitor TeleTracking and EMSTAT for timely placement of patient

14. 14. Collects self-pay, co-pays, or deductibles when indicated by Patient Financial Counseling Department. Issues patients receipt and follows appropriate procedure for securing payment

15. 15. Verifies and reconciles census information with inpatient and ambulatory surgery units and updates change in appropriate system.

16. 16. Prepares admission/ambulatory surgery folders for next days admissions. Reconciles admission lists/OR schedules and investigates discrepancies.

17. 17. Ensures labels and wristbands are made to appropriate areas immediately after admission or transfer.

18. 18. Provides patient and/or family information regarding their admission/surgery. Provides general hospital information in a courteous and caring manner.

19. 19. Follows appropriate downtime procedure; assigning downtime MR# and case numbers, making manual plates; entering correct downtime numbers in system.

20. 20. Obtains accurate information to complete birth certificate and enter into EBRS to DOH within appropriate timeframe; process corrections as indicated.

21. 21. Complete Paternity Form when applicable

22. 22. Enters Height & Weights for Babies in EAGLE for Finance

23. 23. Timely and accurate billing for L&D patients

24. 24. Trained and competent in the use of Forms on Demand (FOD), Electronic Death Registration System (EDRS), Electronic Birth Registration System (EBRS), EMSTAT, PRISM, EAGLE, TeleTracking.

***Requirements:***

* High School Diploma.

Knowledge 0f EPIC patient Access bedding program preferred

Extraordinary customer service skills.

Perfect telephone etiquette
